Error in Book Publishing Wizard

Hi Experts,

We are working on BPC 7.5 M sp04, we are facing an issue "Subscript out of range" while publishing the book via Book Publishing Wizard (as shown in below screenshot), After selecting the book template "Template.xlt" I click on Next button and get the "Subscript out of range" error, please advice what can be causing this error.
