EPM CONNECTOR CONFIGURATION

Hi BPC experts,

I'm on BPC for MS 10 SP 13 and Dashboard Design 4.1 SP2

I've installed EPM Connector but I doesn't know how to configure EPM Connection in Dashboard Design. How  could I fill parameters in below image?
